Cris Formage was first mentioned on the radio talk show hosted by Lazlow Jones in 1992 where he was interviewed. On the show he denied that the Epsilon Program is a cult. Lazlow Jones ends the show by describing Cris as an "alcoholic turned self-deifying cultist".
Cris Formage appears in The Truth mission series for Michael De Santa. Michael receives a call from Cris during Exercising the Truth where he lets him know that he is very interested in his progress. If Michael chooses to steal the money, Cris will call Michael and threaten him for betraying him, but is not seen again in the game.
Cris Formage is seen dressed in what looks like a specially made uniform for the leader of the Epsilon Program. It has a baby blue and white combination just like everything else that is related to the Epsilon Program. He wears a belt with a golden buckle and on his chest is a badge/medal. This outfit is combined with white shoes. His brown hair is gelled and combed backwards. Cris Formage has light blue eyes.

Cris Formage is a clear parody of scientology figures David Miscavige and L. Ron Hubbard, incorporating various elements of both figures' personalities and histories.
The word "formage", if pronounced like how Cris pronounces his last name, is quite similar to "fromage", which is French for "cheese".
On the promotional posters and the in-game website, Cris' skin color is notably lighter and his hair is black. However, when seen in person, his skin color is light brown and his hair color is dark auburn.
