History

Sanji was sent to this island by Bartholomew Kuma during the Sabaody Archipelago Arc. Sanji was found by an okama whose name was Tibany, who treated his wounds. When Sanji regained consciousness, he couldn't see the person who rescued him clearly and thought that it was a beautiful girl. Tibany then asked Sanji if he liked to put on makeup and wear dresses, and Sanji replied positively, thinking that Tibany was determining if he was the right guy for her.

Tibany then rushed towards Sanji, who thought that she was going to jump in his arms but instead, Sanji got knocked down and didn't think that Tibany would have such brute strength. When Sanji finally got a good look at his rescuer, he was horrified to know that Tibbany was actually a man.

The other Okamas of the Island tried to come forward and make Sanji an Okama, telling him that it is a safe place and he need not hide his true self. As Sanji was not interested, he ran away as fast as he could but the Okama didn't stop and went after him to make him one of them. While Sanji was struggling to find a way off the island, an Okama spoke to the Queen and told her about Sanji and about how his kicks did have good strength.

The next day when Sanji was trying to escape the Okamas again, he didn't look where he was going and ran into the Royal Palace and met Carolina. He then asked for a ship so that he could get back with his crew. Caroline agreed, but only if Sanji could defeat the most powerful fighter on the Island, who turned out to be Caroline himself. Sanji would have to abide by the Okama traditions which included wearing a dress, high heels, makeup, etc during the fight.

Sanji attempted to beat Caroline but was unable to land a single attack, as Caroline was ducking from every attack. When Sanji figured out that Caroline was looking under his dress everytime he ducked, Sanji became embarrassed but tried to play it cool, as if he didn't care when someone saw his underwear.

However, Caroline saw this as Sanji's maiden-like modesty coming out. Sanji told Caroline that he didn't know what he was talking about, but after another confrontation, Caroline got the confirmation that Sanji was using his masculinity as a shield to hide his true feelings.

Caroline then used a technique on Sanji that made him question himself about who he really is, and in the end, Sanji was converted into an Okama. Later on, when Sanji read about the Marineford War, it shocked him to the point where he turned back to his normal self.
