Biwako Sarutobi in Naruto

Biwako lived in the newly established Konohagakure and attended the Academy. One day, she fell down and was comforted by Madara Uchiha, whose look terrified her. She was immediately calmed down after Hashirama Senju gave her a flower.

Biwako and her husband Hiruzen Sarutobi met with Kushina Uzumaki and Minato Namikaze prior to Kushina’s due date. Hiruzen decided that it was best for Kushina to give birth in a secure area because she was a jinchūriki. Biwako was put on duty to deliver the baby as she was a medical nin.

On their way to the location, Kushina stopped to speak with Mikoto Uchiha, who recently had a child. Biwako was pleasantly surprised that Mikoto named her kid after Hiruzen’s father, but soon she pushed Kushina to leave and reminded her that the delivery was top-secret. When Kushina went into labor, Biwako had to calm a hysterical Minato while he maintained the seal and she commented that women are far stronger than men. Biwako was later killed by Obito Uchiha after Naruto’s birth.

Biwako appeared to be as smart and intelligent as her husband, providing Kushina sage-like advice on childbirth while also being rather harsh. She also appears to have strong feelings about women, claiming that they are considerably stronger than men because they can go through childbirth while men cannot. She wasn’t above chastising Minato for not focusing on keeping the seal intact during his wife’s childbirth, despite the fact that he was Hokage.

Because of what women go through childbirth, Biwako believes that when a kid is born, the mother should be the first of the two parents to meet the newborn, as shown when Minato wanted to meet Naruto and she turned him away.

Biwako had long hair in a high ponytail, dark eyes and barely apparent creases around her eyes and mouth. She was dressed in a purple kimono-style outfit, a white haori and sandals.

Her hair was shoulder-length with a pink flower hair band when she was a youngster. She wore a green kimono, a light-brown long-sleeved shirt with a loquat on the back, red leggings and shoes.

While little is known about Biwako’s abilities, she held the rank of jōnin, which demonstrated her proficiency. The fact that she was chosen to lead a small team of midwives to deliver the kid of jinchūriki is proof of this. Biwako was also an accomplished medical-nin.

(To Minato) “You're the Fourth Hokage, so stop being so terrified!! If she were a man, she’d have long died from the pain! At least women are strong.”

A. Biwako was a medical-nin and she held the rank of jōnin. Not much of her combat abilities have been known.

A. It hasn’t been specified anywhere who Biwako’s parents were. Her only known family was her husband Hiruzen Sarutobi.
