Eventually, Hashirama was named leader of the Senju Clan and Madara was named the leader of the Uchiha Clan. Izuna fought against Tobirama on multiple occasions. In his final battle with Tobirama, Tobirama caught him by surprise with his Flying Raijin Technique, and landed a fatal blow on him. Seeing his younger brother fatally wounded, Hashirama gave Madara the opportunity to get Izuna to a safe place and Madara retreated with his brother.

Later when Madara clashed against Hashirama in the battlefield again, he revealed that the wounds inflicted by Tobirama on Izuna were fatal and he succumbed to his illness. As his last act, Izuna decided to give his Mangekyo Sharingan to his elder brother so that he could obtain the Eternal Mangekyo Sharingan. He hoped that with this Madara would be able to protect his clan from their enemies, as he had noticed his brother’s blindness was progressing due to overtime use of Mangekyo Sharingan.
