Kokuyo was sent to the Hidden Leaf village the day after the Five Kage Summit alongside Sekiei and Kirara to invite Mitsuki to their secret lab so they could know about the Mitsuki body as he is also an artificial human created by Orochimaru.
As Kokuyo was telling about Lord Ku, the two village gatekeepers, Sao and Uo, noticed them but then Kokuyo and Sekiei attacked them and then left for their lab with Mitsuki. When Sai and Konoha police investigate the scene, they get to know that they have captured Mitsuki, and all of them think that the enemy used Genjutsu on Mitsuki. After learning about the situation, Boruto and Sarada went to rescue Mitsuki against the order from Naruto.
Later on their way to Iwagakure, Kokyuo stopped Sekiei to tell him the plan which lord Ku has for Mitsuki. When Kokuyo got to know that some Konoha shinobi were following them, he told Sekiei to go with Mitsuki as he battled with Konoha shinobi and defeated them, but as he was about to leave to join Sekiei and Mitsuki. Boruto Uzumaki, Sarada Uchiha, and Shikadai Nara found him and engaged in battle.
Kokuyo was impressed by the skill and teamwork of Boruto, Sarada, and Shikadai, but then Chocho Akimichi and Inojin Yamanaka came to help them to fight against Kokuyo. Chocho, Inojin, and Shikadai told Boruto and Sarada to move forward in pursuit of Mitsuki as they would handle Kokuyo by themselves. Still, soon Kokuyo made them retreat as Kokuyo injured Chocho.
Kokuyo then spies on Mitsuki to see if he will help Sekiei or not and when Mitsuki refuses to go back with Boruto, Sekiei is impressed by it and tells him that now he can trust him. Later, while still worried about Mitsuki, those concerns faded as Sekiei's body started to break down.
Kokuyo left Mitsuki and returned to the base for support and after receiving support from Kirara and Kako, they returned to find Mitsuki carrying Sekiei. As Kokuyo and the others became suspicious of Mitsuki, the partially healed Sekiei supported him, saying that Mitsuki came of his own free will with the hope of finding who he truly is.
When he arrived at their headquarters, he talked with Kako about Mitsuki, as the objective was to examine him. Kokuyo, on the other hand, stated calmly that they would wait for orders. Ku had Kako and Kokuyo use more Akuta to look for Onoki after he went missing. Kirara's genjutsu on the village officials ended as her body began to break down.
Ku started an aggressive takeover after realizing they needed to take serious steps to keep their work hidden from the outside world. Once they had succeeded, the mechanical humans became irritated by Ku's refusal to undertake a human heart transplant on them, which would almost definitely save their lives but still not get Onoki's approval.
Later, when Onoki refused to harm any shinobi, Ku got frustrated and took action against Boruto and other Konoha-nin, but somehow they were able to escape. Soon Shikadai and his teammates then encountered Kokuyo, and this time able to defeat Kokuyo with teamwork and one tiny Akuta who saved Inojin. Kokuyo felt ashamed of being defeated by Konoha-Genin and then died.
Kokuyo is a confident individual who just trusts in himself and always gives priority to his mission. He was straightforward with any question and mostly looked down on many shinobi like Konoha-nin. Because of that overconfidence in his ability, he was defeated in the end.
Kokuyo had long black wavy hair and pale skin. The color of his eyes was grey, and he wore bandages all over his body from his mouth to his toe. He wore a purple tunic over a black body suit.
Kokuyo was a superhuman with great physical power and, in one-on-one battles, could defeat most shinobi easily. He has the ability to use Akuta, which is a creature created by an Iwagakure scientists.
Furthermore, he was mentally strong as he was able to break through Inojin's Mind Body Switch Technique in a very short time. He displayed incredible skill with his tonfa, using them like a boomerang, throwing them towards a target, and returning back to his arms.
