Cancer participated in a meeting about Monkey D. Luffy's first bounty, sitting next to Vice Admiral Stainless.
During a meeting in Mary Geoise to discuss Crocodile's replacement in the Seven Warlords of the Sea, Doflamingo manipulated Vice Admirals Mozambia and Stainless into fighting each other using his Devil Fruit ability. Cancer, wanting to stop Mozambia, found himself unable to resist Doflamingo's influence.
Cancer, along with fellow vice admirals, stood ready to fight the Whitebeard Pirates. When Little Oars Jr. joined the battle, Cancer, Doberman, Momonga, and Onigumo faced him. Later, Cancer and Tsuru witnessed the emergence of a coated paddle ship from the bay.
