Not a lot is known about Dr. Kureha’s past but she knows about the Will of D even though the details of it are unknown. When Wapol said that any doctor on Drum Island who does not work under him is a criminal, Kureha and Hiriluk were the only two free doctors left. But unlike Kureha, Hiriluk ended up doing more damage than good to his patients by accident.
When Hiriluk knew that he was on the verge of death, he told Kureha that he needed more time to finish his research on Sakura Powder. Kureha mocked him and said that this cure of his would not heal Drum Island even if it healed him once. While they were arguing, Kureha noticed that Tony Tony Chopper was spying on them. Kureha was confused as to why Hiriluk was in such a hurry while he still had two weeks to live. She finally got to know that the mushroom soup that Chopper had given Hiriluk is actually poisonous and Hiriluk actually had an hour left to live. After finding this out, she smacked Chopper and told him everything which made him cry.
As he had good intentions and did not know what the Jolly Roger sign beside the mushroom meant, Kureha told him that he cannot do anything as he did not have medical knowledge. She also revealed that Hiriluk was at Wapol’s castle so that he could die there and Chopper ran to him. He then drank one of his failed medicines to die and Kureha drank in Hiriluk’s honor. Later, when Chopper went to Kureha, she took him in and vowed that he would become a doctor that is capable of curing any disease. Kureha taught him real medicine for a few years. With Kureha’s help, Chopper developed the Rumble Ball which could modify his devil fruit abilities and gives him four extra forms.
When he took three Rumble Balls he became an uncontrollable monster and went on a rampage. When Chopper woke up in Kureha’s hospital bed he had no memory of what happened once he had eaten the fourth Rumble Ball. After Kureha gives him a lecture to be more careful and that he could be a danger to people around him and that he should never unleash his monster form again.
When Wapol came back to take back his kingdom, the Straw Hats were also on the island looking for a doctor to help Nami. When Wapol tried to destroy Dr. Hiriluk’s flag which was on top of the Drum Castle, Luffy used his devil fruit powers and threw Wapol out of the kingdom. Once Wapol was out of the kingdom for good, the people of Drum Island collectively decided to make Dalton the next ‘King’. After Dalton became the king, he renamed the Drum Kingdom to the Sakura Kingdom so that it could have a new fresh start while erasing all of Wapol’s presence.
Kureha told Chopper that he should go with the Straw Hats so that he can gain all the experience that he needs from the outside world. She gave Chopper a heartful sendoff by setting off pink Sakura snowflakes in the sky.
After the Enies Lobby Arc, Kureha called Dalton to look at the new wanted poster of the Straw Hats and sees that Chopper has a bounty of 50 berries. Dalton says that Chopper’s bounty value is a mistake and Kureha said that she is just happy to see Chopper's face.
In From the Decks of the World Arc, Kureha had taken command of the Ishhi-20 and formed a medical unit at Drum Island. Years later, Kureha increased the Ishhi-20 to 100 while her castle had been turned into a medical center and she had become a head doctor.
During the Zou Arc, Kureha decided to give Dalton company to Mary Geoise as his ship’s doctor despite his protests.
When they got word about Luffy’s Totto Land adventures, Kureha read about it till they reached Mary Geoise. When they reached the castle, the duo had an unpleasant reunion with Wapol and also met Nefertari Vivi. When Donquixote Mjosgard stooped Charlos from capturing Shirahoshi, Kureha stood next to Vivi as Mjosgard promised that he would protect Shirahoshi during the Levely.
Kureha was called a witch by the people of Drum Island due to her ways of treating a patient and threatening them if they do not follow her orders. She also loves to drink and always keeps a bottle close at hand. She is very strict with healthcare and says that medical knowledge and skill are really important for saving patients.
She takes pride in preserving her health and staying in her prime and she misunderstands people who are asking her about the secret of her youth. Kureha also has a loving side to her like how she considers Chopper to be her son. She is really thoughtful and also familiar with the Will of D.
Kureha is a slender woman who has a wrinkled face and she wears a purple blazer with purple low-riding pants with a naval ring. She wears a pair of small sunglasses on her forehead and wears bracelets and rings on her left hand. In the past, she wore her hair in a bunch with a pink tie, a pale yellow shirt with a green line, and a purple dotted line pattern.
While she lived on Drum Island, she wore pants with a pink design and a blazer with a yellow line a white shirt with a pink cherry blossom on the front, and green sleeves and borders. Later on, we see her wearing an orange blazer with a flame pattern on the edge of her right sleeve. She wore the same pants. She is also seen wearing a shirt with a heart.
Kureha has immense medical knowledge and is one of the finest doctors in the world. Her understanding of medicine and the art of healing people is great even among the most advanced doctors. When Nami was bitten and infected by the venerable tick Kestia, which was extinct and as a result, the cure would be challenging to find, she was able to develop an antibiotic to cure her. She is also one of the few medical professionals with a license to utilize NHC10, a highly restricted essence, in the entire world.
She taught Chopper many of her abilities and knowledge, and Chopper went on to become a great doctor in his very own way. After establishing a medical school, she mentored and trained at least eighty students who were qualified to join the Isshi-20, increasing their number of members to one hundred under her guidance. She is also incredibly skilled at determining body temperatures, determining Nami's exact temperature by simply poking her forehead.
In addition to her medical knowledge, she has a very high level of extraordinary strength and speed to match her large lifespan. She showed her ability to carry and throw a large collection of heavy weapons with ease while also easily destroying stone walls. She had incredible tolerance to the cold because she was frequently spotted moving across Drum Island's bitterly cold environment wearing very loose clothing. When both Luffy and Sanji called her old, she could easily beat them up and used powerful kicks to send them flying into walls.
In this technique, Kureha uses physical therapy when she sees a patient about to do something that might affect their healing process and she stops them by using any means necessary. Ecen though her methods are brutal and painful she is always able to stop her patients from doing something dangerous to themselves.
Kureha adopted Chopper as her son. Their first interaction was when she quickly went to Hiriluk's house and was horrified to hear that Chopper had poisoned Hiriluk by feeding him deadly Amiudake, so she beat Chopper badly. She taught Chopper in medical science after reluctantly agreeing to one of Hiriluk's final requests and in response to Chopper's crying pleas. She said to have taught Chopper "everything she knew" and was pleasantly surprised by the young reindeer's ability.
Kureha understands Chopper's past as an outcast among his kind and a monster in human eyes and questioned the Straw Hat Pirates about their ability to heal Chopper's emotional scars, which Luffy managed to do. She used a show of tough love to convince Chopper to leave Drum Island and explore the world, and she bid him farewell with a display of Hiriluk's Sakura fireworks while referring to him as her son. She is overjoyed to learn that he is doing well right now. At the Levely, she is wearing a shirt with Chopper's bounty on it.
Dr. Kureha had an odd friendship with Dr. Hiriluk, she referred to him as a youngster due to her centenarian age, and she seemed to enjoy the company of as a fellow "doctor,". But she regarded him as a quack for his inadequate medical knowledge and unrealistic views that using sakura could save the Drum Kingdom.
The two of them have known each other for a while because Kureha is able to read Hiriluk's personality despite the fact that he is an outcast in the Drum Kingdom.
